Illinois-based Community Health Organizations Awarded $270,000 in Grants by the CVS Caremark Charitable Trust

The CVS Caremark Charitable Trust, a private foundation created by CVS Caremark, has awarded 5 grants totaling $270,000 to community health organizations in Illinois as part of a $5 million commitment to support community health and expand access to quality health care nationwide.

The grants are made possible through the CVS Caremark Charitable Trust’s national partnerships with the National Association of Community Health Centers (NACHC), the National Association of Free & Charitable Clinics (NAFC) and the School-Based Health Alliance.

The Chicago-based health organizations receiving grants are:

·        CommunityHealth (Chicago, IL), in support of a program focused on diabetes management

·        Loyola University Chicago Marcella Niehoff School of Nursing in partnership with a health facility Loyola University Health System (LUHS)-Trinity CHE (Chicago, IL), in support of a mental health outreach and intervention program

·        Erie Family Health Center (Chicago, IL), in support of a program focused on care coordination and patient-centered medical home recognition

·        Near North Health Service Corporation (Chicago, IL), in support of a program focused on diabetes, depression, tobacco-use and HIV/AIDS care coordination

And, Crystal Lake-based Family Health Partnership Clinic will receive a grant to support a program focused on chronic disease management.

“Through our community health partnerships, we are providing much-needed funding to support affordable community-based health care models that help thousands of adults and children on their path to better health,” said Eileen Howard Boone, Senior Vice President of Corporate Social Responsibility and Philanthropy for CVS Caremark and President of the CVS Caremark Charitable Trust. “We are honored to recognize health care organizations in Illinois that are using a variety of innovative methods to help people achieve better health outcomes and provide routine health services to help ensure children and adults have the resources they need to achieve their best health.”

While changes in our health care system qualify millions of more people for health coverage, it's still a challenge for many, especially underserved populations, to have access to quality health care. The CVS Caremark Charitable Trust’s community health partnerships and grants will help ensure that thousands of adults and children throughout the country have the opportunity to benefit from health services right in their local communities that can create better health outcomes overall.
